What is it?

A patient confidentiality and HIPAA authorization form for group visits or virtual group visits; sometimes called shared medical appointments or shared medical visits.

Why do I need it?

Group visits necessarily come with less privacy for the individuals. This provides the patient with his or her rights and responsibilities during the group visit encounter and afterward. It also provides you with the security of knowing you are following HIPAA guidelines while serving your patients in this community format.
