What is it?

A legal contract that sets forth the agreement between a Health Coach and health coaching client.

Why do I need it?

It contains a disclaimer that helps to protect licensed medical professionals that are acting only as a health coach.

It also sets forth the division of liability; explains what is, and what is NOT, included in the services offered; sets forth the rights and obligations of both the client and the coach/company, and explains how and when payment is to be made.

When do I use it?

When enrolling a new health coaching client

Who is it for?

All persons operating as a health coach. 

Licensed medical providers that are NOT operating under their license (e.g., not practicing medicine).
