Why do I need it?
It sets forth the division of liability; explains what is, and what is NOT, included in the services offered; sets forth the rights and obligations of both the patient and practice, and explains how and when payment is to be made.
When do I use it?
Before seeing the patient for the first time.
Who needs it?
All providers operating on a fee-for-service business model (where patients pay for each visit). This agreement is written specifically for doctors who have opted out of insurance and Medicare, and who are not acting as a primary care provider.
