Telemedicine Consent

What is it?

A patient's written consent for the use of telemedicine visits, including acknowledging the risks, benefits, and alternatives to the remote care provided by the provider or their practice.

This consent form is written to comply with state laws and incorporates the recommended provisions in the model policy from the Federation of State Medical Boards (FSMB).

Why do I need it?

It is required in the majority of the 50 States, and is best practice to have it in place in every state. Telemedicine inherently carries unique risks for both standards of care and HIPAA privacy and security breaches.

It adds a layer of protection for your license and your business.

When do I use it?

BEFORE conducting any virtual visits with each patient, whether via Skype, Zoom, or even telephonic visits. Telemedicine is defined differently from state to state, so best practice is to use the written consent form just to be safe.

Who is this for?

All providers that use telemedicine visits as part of their business or practice.
