What is it?
An agreement where the patient agrees to pay for your services, without Medicare limits on how much you can charge and the patient agrees to NOT seek reimbursement from Medicare for your services.

Why do I need it?
It informs and protects both you and your patient from any potential Medicare fraud issues. When you opt out of Medicare, Medicare does not cover services provided by you and Medicare payments cannot be made to you directly or indirectly. Also, Medicare will not make payments to your patients for items or services provided directly by you.

When do I use it?
Before a patient’s first visit (if already a Medicare beneficiary); or before a current patient ages into becoming a Medicare beneficiary.

Who is it for?
All providers who have opted out of Medicare MUST enter into a Private Contract with patients who may be eligible for Medicare benefits.
