What is it?
An agreement where the patient grants you the right to use their testimonial, and some combination of their name (or initials), age, gender, and possibly their image, voice, and/or likeness if using photos or video testimonials.

Why do I need it?
You need legal permission to use somebody’s image and likeness. Getting it in writing is best so you don’t cross any ethical marketing lines and in the event that person, however unlikely, changes their mind down the road.

When do I use it?
If you have a raving fan—or hundreds of raving fans—who want to share their story, make sure you ask them to sign the Consent for Release beforehand.

Who is it for?
All providers who plan on using testimonials in their practice.
